Service Sign-Up & Autopay Forms

To initiate new service (deeded owner only) or enroll in autopay, please select from the forms below. Completing the auto-pay form is optional, but it allows for convenient monthly payments directly from your checking account. For assistance or questions, call 740-477-8255.

 

 

Effective June 3, 2026, the City of Circleville no longer accepts applications to establish utility service in a tenant occupant’s name. All utility accounts must remain in the name of the deeded property owner in accordance with City ordinance.
